Job Summary:

Key Responsibilities:

  • This position will be responsible for case coordination for gender-diverse patients seeking resources for their individual behavioral health needs. Resources can be internal to CHCO and/or external/community-facing. External resources may extend throughout the Rocky Mountain region, based on patients' location. This would include, but not limited to:

  • Collaboration with TRUE Behavioral Health providers to identify patient needs.

  • Meeting with patients and families (in person or virtually) to identify, coordinate, and follow up as needed.

  • Establishing relationships with internal and external behavioral health programs to maintain accurate information.

  • Creating and maintaining handouts and resource lists, as well as internal tracking documents.

  • Other coordination tasks related to patient's behavioral health needs.

Qualifications:

Minimum Qualifications:

  • A bachelor's degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.

  • Two (2) years of professional-level experience in/with medical administration or behavioral health setting, or in an organization related to physical or behavioral health.

  • Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year-for-year basis.

Applicants must meet minimum qualifications at the time of hire.

Conditions of Employment:

  • Must successfully pass a drug test through Children's Hospital Colorado.

  • Must be willing and able to pass a national criminal background check.

Preferred Qualifications:

  • At least one year of experience in care coordination, patient/community outreach,s and/or scheduling.

  • 1 or more years of experience in a clinical setting with adolescents and/or young adults.

  • Experience with Epic is preferred but not required.

  • Any professional or personal experience with transgender and gender-diverse youth.

Knowledge, Skills, and Abilities:

  • Ability to communicate effectively, both in writing and orally.

  • Ability to establish and maintain effective working relationships with employees at all levels throughout and outside the institution.

  • Outstanding customer service skills.

  • Demonstrated commitment and leadership ability to advance diversity and inclusion.

The starting hiring range for this position has been established as $66,537 to $84,635. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.
