WORKING TITLE : Geriatric Medicine Advanced Practice Provider Fellow/Trainee


The Division of Geriatric Medicine is looking for applicants interested in completing a paid 12-month Advanced Fellowship in Geriatric Medicine. The Advanced Fellowship program is funded by The NextFifty Foundation. The program will prepare graduates to enter a career caring for older adults in primary care or skilled nursing facilities with advanced knowledge and experience. Completion of this training opportunity will give graduates the tools needed to care for a very complex patient population independently and with confidence in their future careers. The fellowship will pay $65,000 and will have no wRVU productivity requirements.

This 12-month training program is for graduated and certified NP or PA candidates who desire extensive education and experience in caring for older adults. No previous geriatric medicine experience is required. The training program will include direct patient care supervision/teaching, didactic attendance, and multi-disciplinary experiences caring for older adults. Care settings will include outpatient primary care, subacute rehab, and long term care experiences led by faculty at the University of Colorado, Division of Geriatric Medicine. You will gain knowledge and experience in common geriatric syndromes, complex co-morbid conditions, transition of care coordination, polypharmacy management, advance care planning and excellence in communication methods. In addition, Advanced Practice Fellows will learn side by side with physician trainees, medical students and a variety of other healthcare professionals and provide multi-disciplinary care. Lastly, Advanced Fellow trainees will observe and receive direct training on education and precepting/supervising future healthcare students.

The practitioner will fulfill a variety of roles: direct care provider, coordinator of care, patient/client advocate, health educator, as well as a member of a multidisciplinary care team, or consultant to other health care professionals. The practitioner will hold an Instructor/Fellow appointment in the Division of Geriatric Medicine at the School of Medicine, University of Colorado Denver and have clinical privileges at UCHealth Seniors Clinic, Colorado State Veterans Home, and Aviva, a long-term care and post-acute rehabilitation facility.

Incumbent will report directly to the Program Director, Dr. Skotti Church; supervision by NP, PA and MD faculty during clinical encounters.




Time and effort for this position will be spent in a variety of clinical and classroom learning experiences. Night and weekend call for primary care patients and nursing home residents will be required. Under direct supervision of faculty, trainees will be expected to perform the following functions in addition to didactic educational requirements:

-Performs accurate and complete documentation of clinical encounters

-Billing patient encounters

-Provide comprehensive history, physical and psychosocial assessments of complicated older adults

-Provide consultation in a diverse range of healthcare settings

-Manage and triage patient concerns after normal business hours

-Work with a multi-disciplinary team of professions for care planning
