Job Summary:

KEY RESPONSIBILITIES

  • Attend weekly meetings for oncology services as directed and ensure all new patients are reviewed and assessed for potential fertility preservation consult in a timely manner.

  • Coordinate care with cancer center and external care providers and research partners regarding preservation and interventions. This can include coordination of fertility preservation surgeries at both Children's Hospital Colorado and University of Colorado Hospital, in addition to coordination of egg, embryo, or sperm freezing.

  • Assist with quality improvement measures for the FPRLE program

  • Assist with maintaining a database of all patients for the FPRLE program

  • Assist research team with clinical portions of maintaining QI/research databases with fertility preservation patient and treatment information

  • Monitor data routinely and present outcomes-based measures to the FPRLE program

  • Assist in the continued development of the FPRLE program and improving process flow, which can include: EMR builds, website development, fundraising activities, participating in journal clubs, etc.

  • Maintain 0.5 – 1.0 FTE of outpatient practice, including history and physical examination, procedures, assessments, and planning for benign gynecologic conditions to be balanced with inpatient consultative duties based on the needs of the program.

  • FPRLE patients: conduct outpatient and inpatient consultations. On Call M-F, 8am – 5pm for inpatient consultations at both Children's Hospital Colorado and University of Colorado Hospital. Coordinate patient care, orders diagnostic tests, conveys findings to physician, coordinates patient appointments, and assists with referrals to outside sources. A 10-hour 4 days a week schedule may also be accommodated based on the needs of the program.

  • Leads weekly Clinical Meeting for all FPRLE patients. Review all patient consultations and upcoming surgical cases. Discuss/create a plan for all patients with the Fertility Preservation Program Medical Director.

  • Accessible for FPRLE communication and referrals via email, phone, pager, Secure Chat at both Children's Hospital Colorado and University of Colorado Hospital.

  • Deliver FPRLE Program presentations to local and national populations.

  • Clinical role of the program is inclusive of pediatric male and female fertility preservation in addition to the gynecologic late effects of cancer survivors.

  • Assist with coordinating the transition of patients from active treatment to reproductive health care in survivorship.

Minimum Qualifications:

Instructor:

  • Certified Nurse Practitioner or Physician Assistant

  • 0-2 years of clinical experience

  • Current Colorado NP or PA licensure; Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements)

Sr. Instructor:

  • Certified Nurse Practitioner or Physician Assistant

  • 3 years of clinical experience

  • Current Colorado NP or PA licensure; Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements)

Assistant Professor:

  • Certified Nurse Practitioner or Physician Assistant

  • 5 years of clinical experience

  • Experience as a clinical educator

  • Current Colorado NP or PA licensure; Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements)

Associate Professor:

  • Certified Nurse Practitioner or Physician Assistant

  • 7 years of clinical experience

  • Experience as a clinical educator

  • Current Colorado NP or PA licensure; Basic Life Support certification (Online BLS certifications, those without a skills assessment component are not sufficient to meet the BLS requirements)

Preferred Qualifications:

  • Previous women's health or primary care experience in women's health desired but not required

The CU Fertility Preservation and Reproductive Late Effects (FPRLE) Program within the Department of Ob/Gyn at the University of Colorado Anschutz Medical Campus is seeking a full-time Advanced Practice Provider (APP) to serve as a Fertility Preservation APP. The FP program provides fertility preservation services, education, and comprehensive reproductive health care to pediatric and adult, male and female patients affected by a variety of medical conditions that impact fertility. This is inclusive of medical illnesses or treatments that threaten gonadal function, including oncology, hematology, renal, and rheumatology conditions requiring chemotherapeutic agents. Care is also provided to patients with gonadal dysgenesis or gender dysphoria who will receive treatments that can threaten future fertility. The program serves patients from Children's Hospital Colorado, the University of Colorado Cancer Center, and UC Health. The program APP is responsible for providing inpatient and outpatient consults for these patients throughout the care continuum of pre-treatment counseling through post-treatment survivorship follow-up. The APP will also serve as a resource for referring providers. The APP will endorse consistent communication and foster interdisciplinary teamwork. : Program Coordination / Management Clinical Roles
