Implementation Project Coordinator/Sr. PRA

_Nature of Work:_
The Implementation Project Coordinator is responsible for providing coordination support to ACCORDS/ Department of Family Medicine for a PCORI-funded Diabetes Shared Medical Appointments (SMAs) Pragmatic Clinical Trial. This position will be assisting the Implementation Project Manager and Practice Coach in working with both the study team and practices in implementing two different forms of SMAs at practices throughout Colorado. The Implementation Project Coordinator will engage participating practices in implementing research protocols, aid in implementation of the intervention in practices, support participating practices for diabetes SMA implementation activities, including conducting assessments of practice structures to inform practice redesign and facilitation of the project implementation with clinic staff, with the goal of getting practices ready for participation in the PCORI Diabetes SMA Project. Additionally, this position will provide technical assistance to practices around clinical workflows, development of the shared medical appointment team and implementation of the diabetes education program, as well as data collection, and managing training sessions. This position will also be the liaison between practices and the CU study staff, and will help ensure that research protocols are followed by practices. This is a 1.0 FTE position
This position reports directly to the Implementation Project Manager and receives guidance from the project research team.
_Examples of Work Performed_
1) Project Implementation Technical Assistance and Training
Provide technical assistance and direction to integrated primary care practices and community mental health center health homes in their participation in the Diabetes Project. Facilitate meetings between study staff and practices for designing implementation with the core practice implementation team, providing technical assistance, answering general questions and developing the implementation plan for each practice. Work with other research team members to schedule and coordinate the diabetes education trainings and additional practice facilitation with the Practice Coach.
2) *Practice Facilitator Coordination*
Facilitates and coordinates the planning and implementation of quality improvement and practice redesign in regards to a diabetes group visit model, implementing the study protocol, tracking intervention fidelity, and coordinating practice level data collection. Supports the Practice coach in identifying practice strengths and needs, and works to pool resources to implement a diabetes SMA model. Supports completion of various assessments before starting project implementation and as necessary throughout the life of the project to measure progress. Coordinates practice trainings related to practice redesign, quality improvement and elements of diabetes SMA model and provides technical assistance to clinical team members in regard to the diabetes SMA model implementation. Facilitates the collection of data as needed based on study protocol from practices.
3) Communication and Relationship Management
Establishes, manages and fosters developing relationships among the implementation team members. Identifies issues that may require intervention, and assists practice members with working through issues related to implementation. Is responsible for:
o Moderating discussions and facilitating decision-making and problem solving
o Modeling good communication skills with practice staff
o Supporting sharing of best practices between partners
4) Project Evaluation and Reporting
Supports data collection by research team members for required clinical and process measure reporting. Coordinates study staff practice visits to document observations and maintains detailed notes of practice transformation and implementation activities. As part of the evaluation process, works with the evaluation team to document and measure the process. Provide regular updates to the program team through participation in team meetings. Participates in the publication and presentation of the program’s results.
5) Prepares Study Materials
Under minimal supervision, creates study materials (practice flyers, tracking logs, etc.) and presents them to study team. Creates physical copies of materials for distribution to practices (i.e. training binders, etc.).
Other related duties as assigned by the Implementation Project Manager, Practice Coach, project principal investigators and research study team.

Master’s degree with 3 years relevant experience.
Minimum two years’ experience working in an outpatient primary care or healthcare research setting.
Experience with quality improvement processes (e.g., plan-do-study-act [PDSA]) and workflow redesign.
Strong analytical, problem-solving and decision making skills; ability to independently apply judgment and take action.
Demonstrated proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).
Ability to envision innovative ways of doing things and able to assertively challenge the status quo while maintaining excellent working relationships with team members.
Experience working with community primary care clinics and/or community mental health centers.
Master’s Degree in Nursing, Psychology, Social Work, Communications, Public Health or other related discipline.
Training and/or experience in Implementation Science and/or Practice Transformation/Practice Facilitation and Quality Improvement.
Familiarity with elements of comprehensive primary care and patient self-management support.
Experience in population management.
Experience with team training related to self-management support and team-based care.
Experience in quality data infrastructure and health information technology.
Experience in diabetes prevention and education.
Experience with working with multidisciplinary teams that include physicians, diabetes professionals, behavioral health professionals, and peer mentors.
Work is completed in an ambulatory primary care clinics and community mental health centers, and in administrative office areas of the clinic. Travel to offsite meetings is required up to 25% of the time. Physical requirements of job include prolonged periods of sitting, extensive typing, computer work and light lifting.
